Every Sanctioned All-star player will be required to provide the following and bring to your parent representative. Please use the Completion Form above as a cover and checklist for the items below:

• Parent/Player Completion Form
• Parent/Player Commitment Form (signed)
• Medical Release (signed)
• An ORIGINAL birth certificate*
• Tournament Player Verification Form (signed)*
• Proof of Residence (3 items) OR School Enrollment (1 item)*
• Media Release Form
• Concussion Form

Original birth certificates will be returned once verified by District 35. Hospital Birth Certificates are not recognized as official proof of birth.

Please read residence eligibility document carefully. The documents you provide will need to be within a certain date range.

*Exception: Players who established "residence" or "school attendance" for regular season and/or tournament prior to the 2022 season using the Player Verification Form, and can produce the form with proper proofs and signatures, will be grandfathered in and will NOT need to complete a new Player Verification Form. (Players will still need to provide completion form, commitment form, and medical release).
